Endoskopische Operationen über natürliche Körperöffnungen - das „NOTES-Konzept”: gefährliche Spielerei oder Konzept der Zukunft?

Zusammenfassung

Wohl selten zuvor hat ein neues therapeutisches Konzept in der Gastroenterologie soviel Aufmerksamkeit hervorgerufen wie „NOTES”. Das Konzept des narbenfreien Operierens über natürliche Körperöffnungen ist inzwischen allein schon durch die Berichterstattung in der Laienpresse allgegenwärtig. Die Haltung und persönliche Einstellung zu NOTES reicht demzufolge von grenzenloser Euphorie über skeptisches Abwarten zur bedingungslosen Verteufelung der Methode. In der vorliegenden Übersichtsarbeit wird daher der aktuelle Stand der Forschung - basierend auch auf eigenen Daten - zusammengefasst dargestellt und beurteilt. Während beim Menschen NOTES-Eingriffe bisher nur in wenigen Fällen als laparoskopisch-endoskopische Hybrideingriffe durchgeführt wurden, gibt es bereits viele Daten unterschiedlicher Arbeitsgruppen mit unterschiedlicher Methodik und Indikationen zu NOTES an Schweinen. Das gemeinsame Fazit aller bisher publizierten NOTES-Eingriffe ist, dass derzeit trotz des Reizes der Methode per se noch erheblicher Verbesserungsbedarf besteht. Eine enge Kooperation zwischen Gastroenterologen, Chirurgen und Medizintechnikern ist zur Lösung bestehender Probleme notwendig.

Abstract

Hardly ever before in gastroenterology, a new therapeutical concept has attracted such an interest as NOTES. The concept of scarless surgery via natural orifices is omnipresent. Hence, the attitude towards NOTES ranges from unlimited euphoria to unconditional demonization of that method. This review focuses therefore on the current scientific knowledge - based on additional data of our own. Whereas NOTES in humans has been reported only by case reports in the form of laparoscopic-endoscopic hybrid-techniques, there are several reports of different study groups mentioning different indications and techniques performed in the pig model. The common conclusion of all papers published on NOTES is that despite the appeal of the method per se there is an urgent demand for improvement. Cooperation among gastroenterologists, surgeons and technicians is mandatory to solve these problems.
